Americans with Disabilities Act Site Survey Instrument
FAAST, Inc. Building Accessibility Evaluation Survey - July 1, 2010


FAAST, Inc., in partnership with the Advocacy Center for Persons with Disabilities, Inc. and through support from the Florida Department of Education, Division of Vocational Rehabilitation, has researched and developed a comprehensive Americans with Disabilities Act (ADA) Site Survey Instrument. The 102-page instrument provides general resource and self-help information to assist ADA site survey teams with conducting ADA site surveys of facilities. It contains

  • an introduction
  • a list of survey components
  • instructions
  • a building accessibility evaluation survey
  • 28 separate accessibility forms. 

The ADA site survey includes up-to-date resource information on the ADA Accessibility Guidelines (ADAAG) and the Florida Accessibility Code for Building Construction including hyperlinked references in the on-line forms to applicable ADAAG guidelines and Florida codes.

Additional information is provided on 

  • pre- and post-ADA construction, additions, and renovations
  • program accessibility
  • measuring tips and techniques
  • tools to be assembled before beginning the survey
