The TRAIL Transition Program, Transitioning youth with disabilities into Real vocational opportunities through Action planning and Individualized Learning, is an outcome oriented program whose focus is to assist our 18-22 year old students with disabilities find meaningful, age appropriate vocational placements in our community while keeping in mind their unique learning styles and individual needs.  Our staff helps these young adults identify their dreams and goals before creating their own individualized action plan.

The Transitions School-to-Work Partnership includes the Exceptional Student Education (ESE) and Adult and Community Education (ACE) departments of the Flagler County School District, Flagler Technical Institute, Flagler County Public Transportation, Florida Vocational Rehabilitation, Flagler County Chamber of Commerce, Florida Hospital, and other community businesses, clubs, and organizations. A transition specialist coordinates student schedules. Job coaches, ESE teachers, vocational teachers, and business partners address IEP transition goals of postsecondary education, career and technical (vocational) training, and paid employment through career exploration and training, transportation and life skills training, self-advocacy, and job shadowing.
